& Patient Advocacy Program

Take control of your health care now and in the future.   This program has been
designed with three core tenets: health organization, advanced health care
directives and communication of information.

  • First, to create and organize the health history of individuals to allow for
    quick, comprehensive communication of pertinent health information in
    emergency situations, for new physician appointments, traveling or
    vacationing and other times when medical information may be needed to be
    communicated quickly.

  • Second, to create individual or family advanced health care directives.
    Advanced health care directives communicate your wishes regarding you
    medical care and assist family and loved ones in the very difficult process
    deciding your care and honoring your wishes should you not be able to for
    yourself.  

  • Third, family or patient conference.  At the conference, we will take time to
    review your health history and advanced directive wishes with yourself and
    ANY other individuals you feel need to understand this information.
