1) The Paperwork
Please down download and complete all the following forms bring them to your appointment. These
forms provide us with information that will help us learn more about you and how we will be able to help you achieve your health care goals.
2) A Map to Our Clinic
Please bring this map with you to your first appointment. The layout of our office complex can be a bit confusing and the map makes it much easier to find our clinic.
3) Past Medical Records & Blood Chemistry
Please provide us with any lab work or blood chemistry you have done in the past several years if possible. You can bring this information with you to your initial appointment. Even better… please fax this information to our clinic at 919-848-0211 so we can review prior to your appointment.
Your current medical providers may need your signed request to release your medical information from their office to our office. The following is an authorization for your current medical provider to release records to The Center: Natural Health Specialists.
4) Taking Supplements?
Please bring in all supplements and medications you are currently taking. Nutritional products vary immensely. We will review your current products for potency, purity, active ingredients, and bioavailability to ensure you are taking products you actually need and in dosages that offer the best therapeutic results.
